Additionally, the registered nurse is responsible for the delivery of patient care using the nursing processes of assessing, planning, implementing and evaluating care.
Also directs and guides patient care activities of ancillary personnel while maintaining standards of professional nursing.
Cooperates and communicates well with all health team members to maintain the highest quality patient care.
The Discharge Nurse is responsible for assuring accurate and effective discharge education specifically medication reconciliation and disease.
Additionally, the Discharge Nurse is expected to work intimately with the discharge planning team in fulfilling a safe transition of care.
ResponsibilitiesKey Responsibilities / Essential FunctionsEnsure a complete and accurate discharge medication reconciliation Complete and validate understanding of discharge education / instruction Arrange for follow-up appointments / services Arrange for transportation when appropriate Coordinate with Case Management and/or Social Services to ensure a complete safe transition of care planQualificationsEducation:
Graduate of an accredited school of nursing.
Possesses a nursing license in good standing in the State of Alabama
Experience:
A minimum of two years clinical experience in the specialty setting i.
e.
Med/Surg, Critical Care etc.
Recommended Skills Case Management Certified Nurse Practitioner Clinical Works Critical Care Health Care Nursing Estimated Salary: $20 to $28 per hour based on qualifications.
